Program
Description:
|
Family Housing Network provides a place to live for families
experiencing or at risk of homelessness, while helping to build skills that
maximize family stability and self-sufficiency. Through FHN the Veteran
Homeless Prevention program provides supportive services to Veteran
households at imminent risk of homelessness, or who have become homeless
within the past 90 days. Homeless Adult Services provides 24/7 services to
homeless individuals in Pierce County which includes meals, shelter,
community socialization and support services.

Position Description:
|
The Case Manager is a member of the service team for
Family Housing Network, a housing program of Catholic Community Services.
This position maintains a caseload of formerly homeless families at the
Bridges Village community. This
includes developing and implementing client-defined, goal-oriented Family
Transition Plans, assessing the needs of the families and children, utilizing
resources to meet identified individual needs with the ultimate goal of
maintaining permanent housing and maximizing family self-sufficiency. This
position works closely with the Clinical Supervisor and Housing Services
Director and provides support as requested with the overall objective to
provide outstanding case management to all families in the Family Housing
Network programs. The case manager works closely with the Bridges Property
Manager regarding intakes and ongoing property issues. The case manager also
works closely with the Tahoma Indian Center (a CCS program co-located at the
Bridges Village site) as well as other programs throughout the community in
order to connect families with the resources they need to reach their goals.

Essential
Functions:
|
· Assist
with initial screenings/intakes, assessment of family strengths and needs and
identify support services appropriate for the immediate needs of the family.
· Case
manage families in housing of Family Housing Network.
Ø Perform
required unit inspections and serve as communication link with on-site
manager.
Ø Assist families in
developing a Family Transition Plan and identifying the steps required to
accomplish goals to move the family from homeless to housing stability.
Ø Provide direct
services to families as needed
· Case manager works
closely with FHN case managers and director to address and plan training
opportunities to homeless families in FHN programs and other programs
throughout the community. Support all
FHN programs and case managers.
· Maintain client
files and reports.
· Attend meetings as
necessary and represent Family Housing Network.

Job Requirements:
|
Minimum Qualifications
· BA/BS
in social services or a related field or 3 years experience working in the Human Service/Social Service field.
· Minimum 1 year relevant
case management/housing experience.
· Experience
providing services to low-income/no-income families.
· Experience
in working with and an understanding of Native American culture.
· Ability
and willingness to work in a team environment and promote a positive team
spirit.
· Excellent inter-personnel skills including excellent
oral and written communication skills
· Excellent
organizational and time management skills.
· Ability
to work with a diverse spectrum of high-need individuals and maintain healthy
boundaries.
· Willingness
to meet in individual family homes.
· Applicant must
successfully pass required background checks prior to an offer of employment.
· Must have reliable
transportation, valid driver’s license, automobile insurance and provide
current auto registration.
· Must have an
acceptable driving record per CCS’ driving policy, (required to provide
current driving abstract).
· Proof of negative
TB test upon hire or proof of test within past 12 months.

Preferred
Qualifications
· Experience
with housing services, landlord/tenant law and other housing issues.
· Knowledge of Pierce
County social services and other community resources.
· Strong
computer skills including Microsoft Word and database experience
· Bilingual skills
(English-Spanish).
